Description of Service
Provides support and advocacy services to individuals of any age who have a developmental disability. Services are provided by a nurse and/or medical care coordinator in the client's home. Types of assistance provided: in-home service coordination, information and referral, educational and legal advocacy, home care instruction, health care liaison services, and emotional support.
Referrals can be made by health care professionals, school personnel, community agencies, family members or friends. Permission must first be obtained from the individual or family. Contact the program director or your local unit to make a referral.
Families with children with developmental disabilities, and adults with developmental disabilities diagnosed before the age of 22.
